Volunteer Media Release – Adult

I authorize the Friendly Home to interview me and record the interview; take photographs, videos or films (“Photos”) of me; and use, copy, send and release the interview recordings and Photos for the purpose of advertising, fundraising, marketing or community relations.

I authorize the Friendly Home to send or release the interview recordings and Photos to the news media (newspapers, television or radio), and use, publish and reproduce the interview recordings and Photos of me in print or broadcasts; and on websites, the internet or social media such as Facebook, Twitter, YouTube and Pinterest. I understand I will not be paid for these interviews, photographs, videos or films.

I make no limitations regarding this Authorization. I am signing this Authorization voluntarily.

I understand that this Authorization may be revoked by me at any time by submitting a signed revocation form to the Friendly Home’s Privacy Officer. However, any revocation shall not apply to any action the Friendly Home has taken based on this Authorization.
