Parent/Guardian Consent

I understand that for my son or daughter, to be an employee, intern or volunteer at The Friendly Home, he or she will need to comply with New York State Health Code, which includes proof of Measles / Mumps / Rubella (MMR) vaccines, and proof of a Tuberculin skin test (PPI)) completed in the last year.
